Meet Our Leaders

Dominique Street is a trailblazing healthcare leader and advocate for equitable care, serving as the first Outreach Program Manager jointly established by CommUnityCare for its groundbreaking partnership with the Black Men’s Health Clinic (BMHC)—the first clinic of its kind focused on men of color. In this role, Dominique leads a suite of critical, community-rooted programs that connect individuals to comprehensive care and social support.

She oversees BMHC’s Health Navigator team, clinical and non-clinical student internships, and direct client assistance programs—including payor of last resort services that help individuals access medication, transportation, and essential healthcare regardless of insurance status. Her leadership ensures these programs not only operate efficiently but also with the empathy and cultural awareness needed to truly meet people where they are.

Dominique holds an Associate Degree in Health Administration and is a Certified Medical Assistant, Phlebotomist, Community Health Worker (CHW), Mental Health Peer Support Specialist, and SOAR (SSI/SSDI Outreach, Access, and Recovery) Representative. She also holds certificates in Nonprofit Organizational Development and Grant Writing. Her background spans urgent care, family medicine, behavioral health, and outreach, giving her the credibility to lead with both clinical fluency and lived understanding.

Through her role at BMHC, Dominique helps set a new standard for what equitable, community-centered care can look like. She is known for mentoring with heart, managing with integrity, and designing systems that uplift those who have historically been left out. Her work is expanding what’s possible for integrated outreach and whole-person support across Central Texas.
