BMHC Care Network™

Where Medical and Community Care Come Together

What the Care Network™ Means at BMHC

At the Black Men’s Health Clinic (BMHC), our Care Network™ is more than a list of partners — it’s a trusted, dependable system built to remove barriers and ensure no wrong door to care. Stability, accountability, and trust are the foundation of how we work, and our administrative practices are designed to protect those values.

One of the ways we maintain this consistency is through Memorandums of Agreement (MOAs) with automatic renewal, rather than temporary or informal agreements. This ensures our clients can rely on seamless care, and our partners can focus on solutions — not paperwork.

What it is: A Memorandum of Understanding (MOU) is typically used to outline goodwill or intentions to work together
  • The difference in practice —  Even when the language is similar, an MOU is generally understood as informal — it reflects shared intent but does not carry the same expectation of follow-through. An MOA, by contrast, represents a working agreement: it defines commitments that partners plan to carry out in practice.
Why it’s not ideal for BMHC:
  • Creates uncertainty clients may experience gaps if intentions are not acted upon.
  • Suggests exploration more than execution, which does not align with our Model of Care™, where warm handoffs and dependable navigation require consistent delivery.
At BMHC, our community deserves more than good intentions — they deserve commitments they can depend on.
What it is: A one-year contract that must be re-signed annually. Why it’s not ideal for BMHC:
  • Introduces instability services can lapse just because the paperwork has expired.
  • Shifts focus staff spend time chasing signatures instead of helping clients.
  • Creates hesitation partners may hold back if they know the agreement resets each year.
Our clients need long-term consistency, not administrative disruptions.
  • Clear commitments An MOA is recognized as a true agreement, so both sides know what is expected and how the partnership will function.
  • Stability for clients and partners  Automatic renewal ensures services continue smoothly without interruption.
  • Opt-in / Opt-out model  Partners remain in the Care Network™ unless they choose to step away. This gives us all the freedom to focus on alignment and outcomes rather than annual paperwork.
  • Room to growWith a steady structure in place, BMHC and its partners can refine services, deepen collaboration, and innovate without disruption.
  • For clients You can count on consistent, trusted care without interruptions caused by administrative cycles. Our Care Network™ is built to provide warm handoffs and dependable navigation every step of the way.
  • For partnersJoining BMHC’s Care Network™ means entering into a relationship designed for clarity and stability. Automatic renewal ensures we grow together without the stop-and-start of yearly paperwork.
  • For funders Your investment supports a system built on accountability and long-term impact. The MOA structure ensures that programs continue seamlessly, creating measurable outcomes without unnecessary disruption.
For BMHC, the Care Network™ is about more than partnerships — it’s about trust you can count on. Through MOAs with automatic renewal, we protect clients, support partners, and provide the stability that funders, communities, and families deserve.
