Core Values
At the Black Men’s Health Clinic (BMHC), our Model of Care™ is about more than pointing people toward services — it’s about making sure they’re supported through the process. Health systems are complicated, and people too often fall through the cracks. Our approach is designed to keep that from happening by building a Care Network™ that makes access steady, coordinated, and dependable.
With BMHC, clients don’t just receive a referral. They are connected into a system of medical and non-medical partners who are accountable to each other and to the client. We stay engaged until care is carried through, and when barriers appear, we work with partners and clients to find remedies.
The usual way: Many organizations give out a phone number, brochure, or website and leave the follow-up to the client. What happens next is rarely tracked.
