The Black Men’s Health Clinic (BMHC) exists to eliminate barriers to care for men of color and underserved families by providing no-cost coordination and navigation across medical and social services

Through trusted relationships, community-based support, and culturally responsive guidance, we help individuals connect to the care they need to heal, stay well, and move forward-no matter their income, insurance, or situation

What the Model of Care™ Means at BMHC

At the Black Men’s Health Clinic (BMHC), our Model of Care™ is about more than pointing people toward services — it’s about making sure they’re supported through the process. Health systems are complicated, and people too often fall through the cracks. Our approach is designed to keep that from happening by building a Care Network™ that makes access steady, coordinated, and dependable.

    With BMHC, clients don’t just receive a referral. They are connected into a system of medical and non-medical partners who are accountable to each other and to the client. We stay engaged until care is carried through, and when barriers appear, we work with partners and clients to find remedies.

The usual way: Many organizations give out a phone number, brochure, or website and leave the follow-up to the client. What happens next is rarely tracked.

How BMHC is different:

  • We set appointments and take next steps on behalf of the client, then follow up as needed to keep them comfortable and informed.
  • Care is delivered through our Care Network™ — whether medical providers, housing groups, or social service organizations — so clients move into a coordinated system, not a one-off handoff.
  • Partners and BMHC intentionally keep each other — and the client — updated on both progress and setbacks. If something stalls, it’s flagged quickly, and the Care Network™ works together to remove the barrier before it grows into a bigger problem.

The usual way: One standard intake, one pathway, no matter who you are.

How BMHC is different:

  • Veterans – We help navigate VA and TRICARE alongside local support.
  • Justice-involved clients – We coordinate through courts and reentry partners to address both health and stability.
  • Unhoused neighbors – We embed support with housing and outreach groups so care reaches people where they are.
  • LGBTQ+ community members – We ensure connections to affirming partners who understand identity and choice.
  • Families and community members – Our Care Network™ includes not just clinics, but also food, housing, and job supports.

BMHC’s Care Network™ is built on the belief that care should fit real life, not the other way around.

  • Formal agreements, not intentions: Our Care Network™ runs on Memorandums of Agreement with automatic renewal, so services don’t lapse because of paperwork cycles.
  • Hands-on navigation: Our outreach specialists and health workers take real steps on behalf of each client — from enrollment to transportation to follow-up — so no one is left to manage the process alone.
  • Warm handoffs with accountability: In the Care Network™, warm handoffs don’t end at introductions. Partners and BMHC keep each other — and the client — informed through calls, texts, and in-person check-ins. Progress, setbacks, and even new ideas are shared openly, making sure services move forward smoothly and the client experiences care as one seamless system.
  • Protecting consistency: If a partner isn’t following through, BMHC redirects the client to another Care Network™ provider, steps in directly when needed, and ensures no service stops because of a single breakdown. This built-in safeguard keeps care steady and dependable.
  • For clients: You can trust that you won’t be left alone with a phone number. BMHC and the Care Network™ take action on your behalf and follow up to keep you informed.
  • For partners: Being part of the Care Network™ means clarity, accountability, and the stability to focus on solutions instead of re-signing paperwork each year.
  • For funders: Your investment supports a model that is intentional, steady, and measurable — a Care Network™ that works across health and social supports to make long-term impact possible.
